SUPPLIER QUALITY CLAUSES

MAC 1: First Article Inspections (FAIR) IAW AS9102 to be provided for the initial shipment of any Mod-A-Can part number.

MAC 2: Certificate of conformance shall be provided with each lot. The C of C shall include a statement that the items meet the requirements of the purchase order and/or specifications referenced on the drawing and/or purchase order.

MAC 3: Raw materials certifications including physical and/or chemical test reports shall be provided with each lot. A certification is required for any types of raw materials purchased and/or for the raw materials used to produce items/parts on this order. The supplier is responsible for 100% review of material test reports against the Mod-A-Can drawing and purchase order requirements for which they are intended. The supplier shall maintain reports for 15 years and make them available upon request. These reports shall be traceable to the applicable drawings and specifications for the parts/items on this purchase order.

MAC 4: Mod-A-Can source inspection is required prior to shipment. Mod-A-Can reserves the right to re-inspect and/or retest all items covered by this purchase order. The supplier shall furnish, at no cost, the necessary facilities, equipment, data and perform tests as required by the applicable drawings, specifications and inspection instructions. Final acceptance will be made at Mod-A-Can.

MAC 5: Government source inspection is required prior to shipment. The government reserves the right to re-inspect and/or retest all items covered by this purchase order. The supplier shall furnish, at no cost, the necessary facilities, equipment, data and perform tests as required by the applicable drawings, specifications and inspection instructions. Final acceptance will be made at Mod-A-Can.

MAC 6: Mod-A-Can customer source inspection is required prior to shipment. The customer reserves the right to re-inspect and/or retest all items covered by this purchase order. The supplier shall furnish, at no cost, the necessary facilities, equipment, data and perform tests as required by the applicable drawings, specifications and inspection instructions. Final acceptance will be made at Mod-A-Can.

MAC 7: Right of entry – The supplier shall reserve the right of entry for Mod-A-Can, Mod-A-Can customers and regulatory agency personnel, for access to any and all facilities where work is being performed or is scheduled to be performed, including those facilities of the suppliers sub-tier suppliers, in order to perform inspections.

MAC 8: The Supplier shall have a “Quality System” that is certified to ISO9001-2015, or equivalent.

MAC 9: The supplier shall have a “Quality System” that is certified to AS9100, or equivalent.

MAC 10: The supplier shall have a “Quality System” IAW MIL-STD-45208A.

MAC 11: Flow-down of requirements by supplier to sub-tier suppliers – Where a sub-tier supplier controls critical processes, characteristics of features, the supplier is responsible for control of sub-tier suppliers and assuring that the sub-tier supplier is using the latest process/drawings and meets all purchase order requirements.

MAC 12: Suppliers’ calibration system for the control and maintenance of measuring and test equipment shall be in accordance with ANSI/NCSL Z540-1 or equivalent. The calibration system shall be documented, accountable, and traceable and provide for initial calibration and recalibration during the life of the equipment to traceable standards.

MAC 13: Suppliers are responsible for providing for and implementing configuration control of all changes to deliverable products associated with this purchase order. All changes are subject to Mod-A-Can approval prior to implementation.

MAC 14: All special processes such as: all types of plating, chemical film, heat treating, welding, and painting called out on purchase order shall be performed IAW the specifications and standards called out on drawings and/or this purchase order.

MAC 15: All suppliers that perform any special processes such as: all types of plating, chemical film, heat treating, welding, and painting called out on this purchase order shall be NADCAP approved and have a current certificate of certification that is available to Mod-A-Can upon request.

MAC 16: Limited Life Items – The suppliers certification of compliance is required and shall state the date of manufacture, recommended shelf life, and storage temperature for all items supplied on purchase order. The remaining shelf life of each unit supplied shall be a minimum of 90% of the total shelf life.

MAC 17: Physical and/or Chemical Test Reports – A copy shall accompany all shipments of raw material furnished in accordance with purchase order. The report must specifically identify the material by reference to the melt, cast, heat, drop, lot or other similar designation and must indicate the applicable specification and purchase order number. Test results must state the actual values of the analysis/test.

MAC 18: Raw Material Certification – The supplier must maintain physical and/or chemical test reports covering the raw material supplied or used on order. These reports must be retained for 15 years and be made available upon request. Each shipment shall include a certification that exhibits the applicable specification number, revision and purchase order number.

MAC 19: First Piece Inspection – A copy of the first piece inspection report to be submitted with the shipment of the product ordered on purchase order. The report shall include all the dimensions called out on the drawing(s), including the actual values measured for all quantitative measurements.

MAC 20: Solderability – Assure that parts meet component solderability requirements in accordance with ANSI/J-STD-001 and/or IPC-A-610 Class III or equivalent.

MAC 21: RoHS Compliance – Acting in full accordance with RoHS regulations and documenting testing for RoHS controlled substances. RoHS (Restriction of Use of Hazardous Substances) regulations limit or ban specific substances – lead, cadmium, polybrominated biphenyl (PBB), mercury, hexavalent chromium, and polybrominated diphenyl ether(PBDE) flame retardants – in new electronic and electric equipment.

MAC 22: Date codes of electronic components shall be noted on the Certificate of Compliance (C of C).

MAC 23: Identification of Electronic Devices – Each electronic device or component delivered on all purchase orders shall be identified in accordance with the applicable specification by lot or batch, traceable to the actual manufacturing process and manufacture so as to assure traceability in the event that a defective product condition exists in the lot/batch.

MAC 24: Component Obsolescence Management – Process that addresses all aspects of product life and availability. In the event that a component becomes obsolete or otherwise unprocurable, the supplier’s obsolescence management process shall include provisions for alternate parts, end of life buys, and/or upgraded parts.

MAC 25: Part Substitution – The substitution of parts is NOT AUTHORIZED without Mod-A-Can written approval.

MAC 26: Counterfeit Parts – The supplier, manufacturers, and/or distributors shall reduce the risk of delivering counterfeit parts to Mod-A-Can. The seller shall have written procedures stating that they only procure and sell components from the original Component manufacturers.

MAC 27: Electrostatic Discharge (ESD) Damage protection: Components and assemblies that are susceptible to electrostatic discharge damage shall be handled and packaged to prevent ESD damage. Reference ANSI S20.20 ESD Standard. Containers shall be marked as containing ESD devices.

MAC 28: Foreign Object Debris (FOD) Program – The supplier shall develop and maintain a FOD prevention program to identify and eliminate foreign object entrapment areas and paths through which foreign objects may migrate and cause product failure. Delivered items must be clean and free of machined chips, burrs, grinding dust, forming materials, corrosion, oil and other foreign material on surfaces to prevent FOD entrapment.
